Pre-Appointment Information

Please review all information below prior to your upcoming visit

  • Have you or anyone in your home tested positive for Covid 19 in the last 10 days?
  • Have you had recent exposure to someone who is known to be Covid positive?
  • Are you exhibiting any cold or flu symptoms at this time?

If you responded yes to any of these questions, we kindly ask that you call us immediately at (315) 451-7890 to reschedule your appointment. If you arrive for your appointment with symptoms or recent exposure, we will reschedule your appointment. Thank you!

Before You Arrive

We do require our patients to update Medical History every 12 months or anytime you have changes in your health or medications. To make for the most efficient visit, you may update your medical history prior to your arrival by clicking here.

If your insurance information has changed, please update it PRIOR to arrival. This allows our staff the time to verify your coverage and eligibility as well as any history on file so that we may better estimate your out of pocket costs. If you present new insurance at your visit, you will be asked to pay in full and receive reimbursement from your insurance company. Click here to update your Insurance Information Form.

When You Arrive

Please enter the building to check in at our front desk when you arrive. If you are more comfortable waiting in your vehicle, please just give us a call at (315) 451-7890 to check in.

For the safety of our staff and patients, please note that if you arrive for your appointment and have cold or flu symptoms, you may be asked to reschedule your appointment.

Cancelled or Missed Appointments

Please note that our Cancellation Policy will be strictly enforced at this time. Any appointments cancelled within 48 hours and missed appointments will be subject to our missed appointment fees of $25/30 minutes of scheduled time. Please click to review our Financial and Cancellation Policy.
