We are looking forward to seeing you soon for your dental appointment. Due to the COVID 19 Crisis, you will notice some changes that have taken place in how we are handling patient visits in our practice. We want to make you aware of some of these changes so that you may be prepared when you arrive.
Please call the office at (315) 451-7890 to let us know you have arrived in our parking lot. Do not enter the office until a staff member asks you to do so. Our reception area is closed for the time being to follow guidelines for social distancing.
You will be required to wear a mask that fully covers your nose and mouth when entering and exiting the building. This is per NYS Department of Health Guidelines. Our staff will let you know when to remove your mask and when to put it back on.
Once you enter, a staff member will greet you and take your temperature. You will then be asked a few COVID 19 screening questions. We ask that you answer these questions completely and honestly.
Please come to your appointment alone. If someone accompanies you, they will be asked to remain in your vehicle during your appointment. If you are bringing a minor in for a scheduled appointment, only one parent will be permitted to enter with them. No unscheduled siblings may enter the building at this time.

(Please note: to make appointments most efficient for all of our patients, these forms must be submitted before you enter the building on your appointment date.) Please use these links to pay your estimated out of pocket costs for your scheduled visit. You may do this prior to your appointment or before you leave the treatment room.
We want you to rest assured that we are taking the steps recommended by the CDC and New York State Dept of Health to keep our patients and our staff safe. We appreciate your full cooperation. If for any reason, you are uncomfortable with any of these processes, we ask that you call us immediately upon receipt of this email.
If you have updated insurance information, please use the following link to submit this information PRIOR to your scheduled appointment. We must have your Insurance Company Name, your valid ID number, the name of your employer as it shows on your policy and a Group number. If you are unsure about any of this information, please contact your HR department or your insurance company directly to gather it and then submit the form. If we are unable to verify your benefits prior to your visit, you will be expected to pay for your treatment in full. We will submit your claim for you once your information is received and your insurance company can reimburse you directly.
