Pre-Appointment Information

Please review all information below prior to your upcoming visit

  • Have you or anyone in your home tested positive for Covid 19 in the last 10 days?
  • Have you had recent exposure to someone who is known to be Covid positive?
  • Are you exhibiting any cold or flu symptoms at this time?

If you responded yes to any of these questions, we kindly ask that you call us immediately at (315) 451-7890 to reschedule your appointment. If you arrive for your appointment with symptoms or recent exposure, we will reschedule your appointment. Thank you!

Before You Arrive

Please update your Medical History if it’s been more than 12 months since your last visit or you’ve had any changes in your health or medications.

If your insurance information has changed, please update it PRIOR to arrival. This allows our staff the time to verify your coverage and eligibility as well as any history on file so that we may better estimate your out of pocket costs. If you present new insurance at your visit, you will be asked to pay in full and receive reimbursement from your insurance company. Click here to update your Insurance Information Form.

When You Arrive

Please call the office at (315) 451-7890 to let us know you have arrived in our parking lot or you may enter the building and check in at our front desk. Please note, if there are a high volume of patients in the reception area, you may be asked to wait in your vehicle. You are welcome to wait in your vehicle if this is more comfortable for you.

You will be asked a few COVID-19 screening questions upon check in. We ask that you answer these questions completely and honestly. If you are having any cold or flu symptoms, you will be asked to reschedule your appointment.

Only scheduled patients may enter the building at this time. Minor children may be accompanied by one adult.

Cancelled or Missed Appointments

Please note that our Cancellation Policy will be strictly enforced at this time. Any appointments cancelled within 48 hours and missed appointments will be subject to our missed appointment fees of $25/30 minutes of scheduled time. Please click to review our Financial and Cancellation Policy.
